LIABILITY RELEASE

​

I understand that there are risks of physical injury associated with, arising out of, and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence, arising as a result of such activity from which liability could accrue to April Craig’s Wyldfire Dance Studio, it’s officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities including the Jerome First Presbyterian Church where classes are held (hereinafter collectively referred to as “Wyldfire Dance Studio”).

I hereby agree to release Wyldfire Dance Studio and hold Wyldfire Dance Studio harmless of all liability and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.

I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.

I also give Wyldfire Dance Studio permission to use my child’s picture and/or video in or on any form of advertisement for Wyldfire Dance Studio or a Wyldfire Dance Studio-affiliated event.

If I am a minor, my parent and/or legal guardian has also signed this document releasing Wyldfire Dance Studio from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.

The participant has my permission to participate in Wyldfire Dance Studio events during the 2020-2021 dance season. I warrant the below information is complete and correct. I further release Wyldfire Dance Studio of all liabilities associated with my child’s attendance at Wyldfire Dance Studio.

 

INFECTIOUS DISEASE WAIVER

At Wyldfire Dance Studio, we are following social distancing and cleaning/disinfecting protocols. As with any other public place, we cannot guarantee that your child will not be exposed to COVID-19 or other infectious diseases/contagious illnesses (whether at Wyldfire Dance Studio/Jerome First Presbyterian Church or elsewhere) or, if exposed, that your child will not contract COVID-19 or other infectious diseases/contagious illnesses.

By signing below, you agree to waive any claims of liability against Wyldfire Dance Studio, April Craig, and Jerome First Presbyterian Church and to hold harmless Wyldfire Dance Studio, April Craig, and Jerome First Presbyterian Church if your child contracts COVID-19 or other infectious diseases/contagious illnesses despite our best efforts to prevent it.

Additionally, I agree to notify April Craig and Wyldfire Dance Studio if my child or any member of our household contracts COVID-19 or other infectious diseases/contagious illnesses or has come into contact with someone who has tested positive for COVID-19 or other infectious diseases/contagious illnesses. This action will help ensure the utmost safety of our dancers, families, and staff.
